Claim Procedure for Health Insurance

CLAIM PROCEDURE IN CASE OF CASHLESS HOSPITALISATION

  1. Patient gets admitted to hospital.
  2. Insurance company will review the request.
  3. Insurance company will inform the hospital with the authorization status.
  4. Insurance company will give approval only for medical expenses.
  5. Payment of bills will be made to the concerned hospital.

CLAIM PROCEDURE IN CASE OF RE-IMBURSEMENT

  1. Patient gets admitted to hospital, takes treatment and pays the hospital bills.
  2. Intimation to concern insurance company for registering claim on the Toll Free No. or through mail within 24 hrs of hospitalization.
  3. Customer submits all original documents to Jaika Insurance brokerage Pvt. Ltd. Along with duly filled claim form after discharge.
  4. If the claim is under query, a query letter is sent for submission of required documents.
  5. If the claim is approved, payment will be made to the customer as per the policy terms and conditions.
Note :- Required 24 hrs. hospitalization in both the cases.

DOCUMENTATION FOR CASHLESS HOSPITALISATION:

  1. Cashless Card.

DOCUMENTATION FOR RE-IMBURSEMENT:

  1. Duly filled claim form with treated doctor’s sign and hospital stamp.
  2. Policy Copy.
  3. Original Hospitalisation Documents, like discharge card, bills etc.
  4. Canceled cheque.
  5. Ecs Form.
